We encourage you to contact your insurance carrier to ensure benefit eligibility. Occasionally mental health coverage can be different from your standard coverage.

**Please note that as of July 1, 2023, Anthem will no longer be accepted for medication management. Our prescribers are no longer in network with Anthem. Anthem will continue to be accepted for psychotherapy.

Fee for Service

Self-pay is accepted
Rates vary based on the service type (Intake, standard session, talk therapy, medication management, etc)

Forms of Payment

Cash, card, and/or check are accepted
Payments are due at the time of service. This includes any co-pays and/or deductible plan payments.
A required form is included in the paperwork for new patients to provide information for a card to be kept on file
Unpaid balances may be cause for discontinuation of services.
